
Driver Medicals
If you’re applying for or renewing a Group 2 licence to drive lorries (HGVs), buses or minibuses (PCVs), you’ll need to complete a D4 Medical to show that you meet the medical standards set by the DVLA.

What’s included in a D4 Medical:
• Vision test (with or without glasses/contact lenses)
• Blood pressure check
• Medical history review – Screening for conditions such as diabetes, heart issues, epilepsy, and mental health • Completion of the D4 medical form ready for DVLA submission
When is a D4 Medical Required?
• When applying for your first Group 2 licence
• At age 45, then every 5 years until age 65
• Annually from age 65 onwards
• If requested by the DVLA due to medical concerns

Please bring the following with you:

Before attending, we would be grateful if you could:
Complete the ‘Your Details’ (Applicant) and ‘Your Doctor’s Details’ sections on page 1
Fill in your name and date of birth on the bottom of every page
Bring the completed form with you to your appointment
You will also need to bring:
• Your driving licence
• A list of any medications you’re taking or details of any hospital treatment
• If you have diabetes or heart problems, a printout from your GP with relevant health information
• If you wear contact lenses, please remove them 24 hours before the appointment and bring your glasses with you
